People with aphasia use gestures not only to communicate relevant content but also to compensate for their verbal limitations. The Sketch Model (De Ruiter, 2000) assumes a flexible relationship between gesture and speech with the possibility of a compensatory use of the two modalities. In the successor of the Sketch Model, the AR‐Sketch Model (De Ruiter, 2017), the relationship between iconic gestures and speech is no longer assumed to be flexible and compensatory, but instead iconic gestures are assumed to express (...) information that is redundant to speech. In this study, we evaluated the contradictory predictions of the Sketch Model and the AR‐Sketch Model using data collected from people with aphasia as well as a group of people without language impairment. We only found compensatory use of gesture in the people with aphasia, whereas the people without language impairments made very little compensatory use of gestures. Hence, the people with aphasia gestured according to the prediction of the Sketch Model, whereas the people without language impairment did not. We conclude that aphasia fundamentally changes the relationship of gesture and speech. (shrink)

Zusammenfassung Patientinnen und Patienten steht das Recht auf Behandlung nach ihren eigenen Vorstellungen auch dann zu, wenn sie aktuell keinen eigenen Willen bilden können. Advance Care Planning (ACP), als ein spezielles Verfahren der gesundheitlichen Vorsorgeplanung, zielt darauf ab, dieses Dilemma durch eine Willensbestimmung im Voraus aufzulösen. Besonders ausgebildete Gesprächsbegleiter*innen bieten an, bei der Ermittlung, Formulierung und Dokumentation eines solchen, die individuelle gesundheitliche Situation berücksichtigenden Willens zu helfen. Das Umfeld der Betroffenen soll in den Gesprächsprozess einbezogen und es soll organisatorisch gesichert (...) werden, dass deren Willensbestimmung den aktuell Behandelnden im Bedarfsfall faktisch zugänglich ist. Das Verfahren kombiniert also ein kommunikatives mit einem institutionellen Element. Aus ethischer Sicht stellen sich dabei zwei wesentliche Herausforderungen. Erstens erfordert Gesprächsbegleitung mit einem so komplexen Ziel Kompetenzen und setzt bestimmte Haltungen voraus, wie sie für Professionen im strengen Sinne typisch sind. Daher wäre es wichtig, entsprechende professionelle Normen explizit zu machen, beispielsweise in Form eines Berufs-Kodex. Zweitens muss das Verfahren trotz seiner institutionellen Form für die Betroffenen freiwillig sein. Dies ist entscheidend für die Validität des Prozesses, aber auch für dessen Ergebnis. Denn wenn Letzteres rechtlich bindend sein soll, müssen die Betroffenen freiverantwortlich, d. h. ohne sog. Willensmängel handeln können. Es ist deshalb ethische Pflicht, beim Eintritt in und im Verlauf der Beratung auf die Freiwilligkeit zu achten. (shrink)

Il cosiddetto “paradosso della finzione” nasce dal tentativo di spiegare quale tipo di emozioni proviamo verso quegli oggetti che troviamo nei romanzi e nei film e che sappiamo perfettamente essere fittizi. Si tratta di un paradosso classico, tornato alla ribalta nel 1975 dopo la pubblicazione di un articolo di Colin Radford che partiva precisamente dall’interrogativo riguardante le lacrime che versiamo per ciò che non è reale (come un personaggio fittizio, appunto). Perché ci commuoviamo per il suicidio di Anna Karenina se (...) sappiamo che non esiste? Che differenza c’è tra piangere per Anna Karenina, piangere per una bugia, piangere per la depressione e fare finta di piangere perché si vuol fare credere a chi ci guarda che siamo sinceramente commossi? In Piangere e ridere davvero (2009) Maurizio Ferraris offre un’interessante proposta teorica per dissolvere il paradosso in questione e si sofferma su alcune distinzioni fondamentali riguardanti le nostre reazioni emotive a stimoli reali e fittizi. In questo saggio si prende spunto dalla teoria di Ferraris e ci si propone di chiarire che cosa significhi fare finta di rispondere emotivamente a qualcosa nella vita di tutti i giorni e quale sia la differenza tra queste “finte” emozioni e quelle che invece proviamo durante la fruizione di opere che sappiamo essere di finzione. Saranno scelti come esempi paradigmatici il caso Fenaroli (al centro della cronaca italiana alla fine degli anni Cinquanta), Il vedovo di Dino Risi e Scusate il ritardo di Massimo Troisi. (shrink)

El problema del valor cognitivo de las obras literarias ha recibido fundamentalmente dos tipos de respuesta, la cognitivista y la anti-cognitivista, la primera sostiene que la literatura es vehículo de verdades universales y la segunda mantiene que no transmite nada salvo falsedades y trivialidades. Entre estas posiciones opuestas, este ensayo defiende una forma de cognitivismo débil afirmando por una parte, con los cognitivistas, la idea de que aprendemos de la literatura, y por otra parte, con los anti-cognitivistas, la intuición de (...) que la literatura no nos transmite verdades sobre nosotros mismos o sobre el mundo. La idea es que una solución basada en dos niveles, de acuerdo con lo que aprendemos de la literatura, se deriva de una combinación de knowing that y knowing what. En primer lugar, para aprender algo de la literatura necesitamos comprender, y por tanto saber, de qué hablan los textos literarios, tenemos que ser capaces de aferrar el estado de cosas descrito. Este es el primer nivel, relativo a la verdad literaria y al knowing that. En un segundo nivel, nuestro aprendizaje a partir de los textos literarios tiene que ver con imaginar cómo es ser esos personajes involucrados en esos eventos, por lo tanto, knowing what es como vivir una vida diferente de la propia. En este nivel aprendemos de la literatura del mismo modo en que aprendemos de los experimentos mentales, ejemplos o contraejemplos. Background Although simulation-based learning using simulated patients is a standard part of training in medical school, it is not yet used to the same extent in the teaching of medical ethics. There are good reasons to use simulation-based teaching, especially in clinical ethics, to gain practical experience through the situation-specific combination of knowledge, skills, and attitudes in the learning process. However, there are certain prerequisites regarding the design of simulations with actors in medical ethics education. Topics Using a concrete example, (...) this article aims to provide an overview of the development and conception of simulation and role scripts for simulations with actors to teach clinical ethics, which is an important subfield of medical ethics. The special requirements and specifics of these simulations are addressed. Conclusion Although there are some limitations with regard to integrating simulations into clinical ethics, simulation-based training of knowledge, skills, and attitudes can and should play a role in medical–ethical theory and clinical–ethical practice, not only for medical and nursing students but also in the further training of clinical ethicists. Parkinson's disease patients receiving dopaminergic treatment may experience bursts of creativity. Although this phenomenon is sometimes recognized among patients and their clinicians, the association between dopamine replacement therapy in PD patients and creativity remains underexplored. It is unclear, for instance, whether DRT affects creativity through convergent or divergent thinking, idea generation, or a general lack of inhibition. It is also unclear whether DRT only augments pre-existing creative attributes or generates creativity de novo. Here, we tested a group of PD patients (...) when “on” and “off” dopaminergic treatment on a series of tests of creative problem-solving, and related their performance to a group of matched healthy controls as well as to their pre-PD creative skills and measures of inhibition/impulsivity. Results did not provide strong evidence that DRT improved creative thinking in PD patients. Rather, PD patients “on” medication showed less flexibility in divergent thinking, generated fewer ideas via insight, and showed worse performance in convergent thinking overall than healthy controls. Pre-PD creative skills predicted enhanced flexibility and fluency in divergent thinking when PD patients were “on” medication. However, results on convergent thinking were mixed. Finally, PD patients who exhibited deficits in a measure of inhibitory control showed weaker convergent thinking while “on” medication, supporting previous evidence on the importance of inhibitory control in creative problem-solving. Altogether, results do not support the hypothesis that DRT promotes creative thinking in PD. We speculate that bursts of artistic production in PD are perhaps conflated with creativity due to lay conceptions of creativity. (shrink)

La inmunización en adultos es una estrategia de salud pública que se ha desarrollado e impulsado en los últimos años como consecuencia del envejecimiento poblacional. El aumento de la proporción de personas adultas en la población y la manifiesta preocupación por promover un envejecimiento saludable de la población plantean la premura de aportar evidencia científica sobre los factores que se asocian a la vacunación de adultos. La prevalencia de inmunizaciones en adultos está por debajo de las metas planteadas por el (...) Ministerio de Salud, por ello se propone como objetivo explorar e identificar los factores asociados a la vacunación de adultos en Argentina, teniendo en cuenta las recomendaciones del calendario nacional de inmunizaciones. Se trata de un estudio cuantitativo, exploratorio y correlacional, cuya fuente de datos es la Encuesta Nacional de Factores de Riesgo de 2013. En la metodología se desarrollan modelos logísticos individuales usando como variables explicativas sexo, nivel de instrucción, cobertura de salud, situación conyugal y si accedió a información sobre vacunas de adultos en algún medio masivo de comunicación. La unidad de análisis son las personas mayores de 18 años y se analizan las subpoblaciones objetivo de las siguientes vacunas: doble bacteriana o triple acelular del adulto, influenza y neumococo. Como contribución puede mencionarse que los resultados indican que haber visto o escuchado información sobre vacunas de adultos resultó el factor que más aumenta las probabilidades de estar inmunizados de los adultos argentinos. Una forma frecuente de medir y estimular el desarrollo narrativo infantil utiliza el recontado de una narración. En Chile, se han diseñado tareas considerando la estructura del input verbal, tanto en un nivel léxico-sintáctico como textual, estableciéndose –incluso– niveles de complejidad. En el marco del Proyecto FONDECYT 1130420, para elaborar un cuento infantil –input de la tarea de recontado– se hizo hincapié no solo en los rasgos verbales, sino también visuales del relato. De tal forma, se propuso un doble objetivo: (...) diseñar las imágenes de una historia potenciando su significado a través de una gramática visual y observar en qué medida estos elementos aparecen reflejados en el recontado oral de los sujetos. Para este propósito, junto a la Gramática de la Historia para el discurso, se consideró la Gramática de Diseño Visual y sus herramientas analíticas para la construcción de imágenes. El cuento se piloteó en una muestra de 20 pre-escolares de Kinder de un colegio particular subvencionado de la Región de Valparaíso. Sus discursos fueron videograbados, transcritos y analizados. Se observó que todas las imágenes narrativas aparecen mencionadas en más de un 60% de los recontados. Además, existen tres referidas en el 90% de los discursos infantiles y representan momentos cruciales del relato o cierta complejidad del input como la simultaneidad de acciones. Estos resultados permiten valorar la importancia de los elementos visuales del cuento en la tarea de recontado infantil. (shrink)

Tierschutz neu denken: Würde – nicht nur Leid vermeiden.

Seit über zehn Jahren moderieren die Autor*innen mit hohem Zuspruch das Ethik-Café in Einrichtungen des Sozial- und Gesundheitswesens. Menschen aus unterschiedlichen Berufen, Patient*innen, An- und Zugehörige sowie ethisch Interessierte haben im Ethik-Café die Möglichkeit, aus ihrer persönlichen und beruflichen Perspektive ethische Fragestellungen zu diskutieren, die für ihren Alltag relevant sind. Ethik-Cafés sind niederschwellig, multiprofessionell, interdisziplinär. Die Teilnehmer*innen profitieren von dieser Perspektivenvielfalt. Das Buch stellt die Konzeption sowie deren Evaluation, die theoretischen und methodischen Voraussetzungen und in vielen Beispielen die praktische Umsetzung (...) von Ethik-Cafés dar. Das entwickelte Vier-Phasen-Modell des ethischen Prozesses wird exemplarisch in 25 Ethik-Cafés zu Themen aus dem Sozial- und Gesundheitswesen angewendet und umgesetzt. In aging humans, aerobic exercise interventions have been found to be associated with more positive or less negative changes in frontal and temporal brain areas, such as the anterior cingulate cortex (ACC) and hippocampus, relative to no-exercise control conditions. However, individual measures such as gray-matter (GM) probability may afford less reliable and valid conclusions about maintenance or losses in structural brain integrity than a latent construct based on multiple indicators. Here, we established a latent factor of GM structural integrity based (...) on GM probability assessed by voxel-based morphometry, magnetization transfer saturation, and mean diffusivity. Based on this latent factor, we investigated changes in structural brain integrity during a six-month exercise intervention in brain regions previously reported in studies using volumetric approaches. Seventy-five healthy, previously sedentary older adults aged 63–76 years completed an at-home intervention study in either an exercise group (EG;n= 40) or in an active control group (ACG;n= 35). Measures of peak oxygen uptake (VO2peak) taken before and after the intervention revealed a time-by-group interaction, with positive average change in the EG and no reliable mean change in the ACG. Significant group differences in structural brain integrity changes were observed in the right and left ACC, right posterior cingulate cortex (PCC), and left juxtapositional lobule cortex (JLC). In all instances, average changes in the EG did not differ reliably from zero, whereas average changes in the ACG were negative, pointing to maintenance of structural brain integrity in the EG, and to losses in the ACG. Significant individual differences in change were observed for right ACC and left JLC. Following up on these differences, we found that exercising participants with greater fitness gains also showed more positive changes in structural integrity. We discuss the benefits and limitations of a latent-factor approach to changes in structural brain integrity, and conclude that aerobic fitness interventions are likely to contribute to brain maintenance in old age. At Universitat Politècnica de València, Meridies, an internship programme that places engineering students in countries of Latin America, is one of the few opportunities the students have to explore the implications of being a professional in society in a different cultural and social context. This programme was analyzed using the capabilities approach as a frame of reference for examining the effects of the programme on eight student participants. The eight pro-public-good capabilities proposed by Melanie Walker were investigated through semi-structured interviews. (...) The internship is an environment in which students can put into practice the knowledge they have acquired in undergraduate studies and to find practical relevance in what they studied. Occasionally, this also entails a critical questioning of what they have learned, a greater awareness of the limits of the contents of their studies and of the way things were taught, and interest in less explored issues that are closely linked to social justice. However, tensions can arise between the pro-public-good oriented perspectives of this programme, and a more instrumental vision. One way to overcome these tensions is to foster consideration of reflexivity, that is, the dynamic relationship between technology and society. To do so, the programme must create space before and during the internship, and upon the return of the students, to discuss and collectively reflect upon their lived experience. Additionally, it ought to engage supervisors in this educational journey, both at the university and in the host institutions, and also involve socially committed organisations in this task. Este artículo sintetiza algunos aspectos del proyecto “Hierro y bronce. Criterios para la valoración y conservación de la herrería artística en el patrimonio arquitectónico del Uruguay”[i], cuyo objetivo es establecer los atributos patrimoniales de los elementos ornamentales metálicos asociados a fachadas del patrimonio arquitectónico nacional y contribuir a su difusión, puesta en valor y conservación. A lo largo de la investigación se abordan los aspectos socioculturales, artísticos, formales, técnicos y, por último, los relativos al deterioro de estos elementos y las (...) pautas para su preservación y restauración. La metodología se basa en el estudio de fuentes primarias, éditas e inéditas, en la revisión de bibliografía especializada y en el trabajo de campo sobre una muestra representativa de elementos ornamentales metálicos aplicados a las fachadas de la arquitectura montevideana. Fue relevado un total de 250 edificios, correspondientes a los distintos períodos que distinguimos en la evolución de la herrería local entre finales del siglo XVIII y mediados del siglo XX. Se trata de un aporte inicial que profundiza en un campo escasamente abordado en nuestro medio acerca del conocimiento y difusión de los trabajos de herrería en el marco del patrimonio arquitectónico y artístico uruguayo, proporcionando información útil a la academia, las instituciones vinculadas a la valoración y conservación patrimonial y el público general. En busca de comprender los cambios en la fecundidad en el Noroeste argentino, este estudio tiene por objetivo analizar su comportamiento entre 2015 y 2020. La investigación se justifica por la relevancia de examinar cómo ha evolucionado en los últimos años en la región. Mediante un análisis cuantitativo y descriptivo, a partir de los datos obtenidos del Departamento de Estadísticas e Información de la Salud (DEIS), se calcularon Tasas Específicas de Fecundidad (TEF) y Tasas Globales de Fecundidad (TGF). Las TGF (...) fueron categorizadas entre aquellas que se situaron por debajo de 2.1 (nivel de reemplazo), 1.6 (nivel de baja fecundidad) y 1.3 (nivel de muy baja fecundidad) hijos en promedio por mujer. Se aplicaron los modelos de baja fecundidad que la ONU propuso en 1963 para analizar las TEF, identificando tres regímenes de fecundidad: temprano, dilatado y tardío. Se observó el cambio en las TEF de 15 a 19 años y su relación con el resto de los grupos etarios. Los resultados revelan que, durante el periodo 2015-2020, la fecundidad en el promedio regional y en las provincias del NOA experimentó un pronunciado descenso; que las TGF se situaron por debajo del umbral mínimo de reemplazo, e incluso en algunos casos alcanzaron niveles de baja y muy baja fecundidad, oscilando entre 1.6 y 1.3 hijos en promedio por mujer; y que se produjo una concentración de los nacimientos en el grupo de mujeres de 25 a 29 años. Kant nimmt in der Einleitung zur Metaphysik der Sitten eine Einteilung in innere und äußere Gesetzgebung vor. Beide leitet er aus demselben in der Grundlegung zur Metaphysik der Sitten formulierten kategorischen Imperativ ab, und fügt Letzterem weitere Elemente einer Grundanthropologie hinzu, die das Material für die einzelnen Teile der Rechts- und Tugendlehre darstellen. Die Zusammenhänge zwischen Rechts- und Tugendlehre sind aber komplexer als die Einteilung in äußere und innere Gesetzgebung es nahelegt. Sie teilen etwa dieselben Anwendungsmethoden und Metaphern, und bei (...) einer näheren Untersuchung der einzelnen Teile erweist sich diese Einteilung als entweder problematisch oder unscharf. In der Textinterpretation der einzelnen Beiträge kommen die genannten Zusammenhänge und Beziehungen zum Vorschein – insbesondere weil ein Teil der Autoren dieses Bandes zum Kommentar beider Werken beiträgt. Ein weiterer Schwerpunkt dieser Textinterpretationen liegt bei der kritischen Untersuchung des Ertrags dieser Werke für die entsprechenden heutigen Debatten und die Auseinandersetzung mit heutigen im weiten Sinne „Kantischen“ Positionen. Durante la edad escolar, los sujetos van logrando una madurez sintáctica, evidenciada en el incremento de lacomplejidad estructural de los textos que producen. Diversas teorías han dado cuenta de dicha complejidad(Hunt, 1970; Katzenberger, 2004; Nir & Berman, 2010), sosteniendo que la misma está vinculada con eltipo de relaciones interclausulares dentro del texto. Asimismo, corroboran que dicha complejidad no sólo seincrementa con el nivel educativo, sino también está guiada por la modalidad discursiva. A partir de estapropuesta, el proyecto FONDECYT 1100600, se (...) planteó el objetivo de describir el desarrollo de la sintaxisoral, considerando otra perspectiva de la complejidad: la diversidad interclausular. Para ello, se aplicarondos tareas de elicitación (narración y explicación), a 250 estudiantes de kinder, 3º y 6º básico, Iº y IVº mediode Valparaíso y Santiago. Luego de ser transcritos y analizados, se reconocieron en los textos las relacionesinterclausulares y se estableció un índice de diversidad clausular, en el entendido de que a medida que seusaban más formas para relacionar las cláusulas, mayor sería la complejidad sintáctica de los mismos. Losresultados muestran un incremento de la diversidad clausular a medida que aumenta el nivel educativo, sinembargo, su significatividad se da sólo entre algunos niveles. Además, entre ambos tipos de textos no semanifiestan grandes diferencias, salvo en Kínder y en IVº Medio. Estos hallazgos permiten vislumbrar que lamadurez sintáctica implica tener mayor cantidad de estructuras gramaticales y utilizarlas estratégicamentepara construir un determinado texto. (shrink)
